MP/H Rules--Colon: Please review the following and advise on what code should be used for primary site and histology.

Preoperative diagnosis: familial polyposis with rectal and rectosigmoid cancer.

Path report from colon resection:

Tumor site: sigmoid Tumor size: 5mm Histology type: adenocarcinoma.

Tumor site: distal rectum Tumor size: 1.9cm Histology type: adenocarcinoma with signet ring cell differentiation. Tumor site: proximal rectum Tumor size: 4.0cm Histology type: adenocarcinoma.

The gross descrption states: The mucosa of the colon is tan pink with polypsis throughout; more than 1000 tan sessile polyps.

Are we correct in coding this case as a single primary using MPH rule M3 with histology of 8220/3 (familial polyposis) using MPH rule H17 with the primary site of C199?

Answer
Assign C199 and 8220/3 for this case.

For cases of familial polyposis, when the rectum is involved, assign C199 (colon and rectum). When the rectosigmoid or rectum is not involved, assign code C189. H17 is the most applicalbe histology rule for this case.
