Manual Revisions
This is a summary of the changes in the 2012 Hematopoietic Manual. Changes such as correcting typographical errors, breaking one of the modules into two modules, and updating the notes, “This is a change from previous rules” to “This is a change from pre-2010 rules”, are not included in this outline.
Preface to Manual: Three new paragraphs
- Transformations: Chronic Neoplasm to Acute Neoplasm
- Steps in Priority Order for Using the Heme DB and Hematopoietic Coding Manual
- Other Treatment: Reporting Phlebotomy, Blood-Thinners/Anti-Clotting Medications, and Transfusions as treatment
Multiple Primary Rules
- Note 1 preceding rules (explaining the definition of chronic and acute neoplasms)
- M3 Sarcomas and leukemias with the same lineage
- M4: Multiple MDS subtypes (DB shows this has been the practice of registrars, but we didn’t have a rule to support that practice)
- M5: FL and DLBCL (added upon request. M rule to correlate with PH rule for FL and DLBCL)
PH Rules
- Instructions for coding histology and primary site taken out of Module 1. Instructions now precede the rules.
- Addition: Instruction 5: Do not use ambiguous terms to code a specific histology. Only change/addition (previously in educational presentations only)
- Module 1: PH1 and PH2 Coding post-transplant lymphoproliferative disorder. NEW (based on frequent registrar questions)
- Module 3: List corrected. Obsolete codes replaced with current codes. Revised
- PH8: CLL/SLL Revision
- Module 4: Sarcomas and leukemias with the same lineage (correlates with M3). NEW
- PH21: Added words synchronous/simultaneous. Revised (based on registrar questions)
June 2010, Version 1.6
Database Revisions
- 9985/3: Had an ICD-10 code of D46.71. Corrected to D46.7.
- 9986/3: Had an ICD-10 code of D46.73. Corrected to D46.7.
- 9591/3: Non-Hodgkin lymphoma, NOS: Deleted the term "lymphosarcoma" from alternate names.
- 9967/3: Typo in alternate name "zMyeloproliferative variant of the hyperesinophilic syncrome" changed to "Myeloproliferative variant of the hyperesinophilic syndrome.
- 9591/3: Capitalized the word Burkitt in, "Malignant lymphoma, undifferentiated cell, non-burkitt."
- 9596/3: Deleted the term "Composite Hodgkin and non-Hodgkin lymphoma" from alternate names since the preferred term was repeated as an alternate name.
- 9733/3: Plasma cell leukemia: deleted "plasma cell dyscrasia" as a synonym.
- 9872/3: Added the term "Acute myeloid leukemia with minimal differentiation" and checked the box to make Acute myeloid leukemia with minimal differentiation the preferred term.
- 9805/3: Acute biphenic leukemia: entered site code C421 in primary site box.
- 9814/3: Changed preferred name from (P13;22) to (p13;q22). Added q before 22.
- 9869/3: Changed preferred name from (q21p26;2) to (q21p26.2). Changed the 26;2 to 26.2.
- 9827/3: Adult T-cell leukemia/lymphoma Treatment: Immunotherapy listed twice, deleted one of the entries.
- 9761/3: Waldenstrom macroglobulinemia, Abstractor Notes: Deleted last paragraph.
- 9690/3: Follicular lymphoma, NOS Abstractor notes, first paragraph. Changed "follicular lymphoma, grade;," to "follicular lymphoma grade 2;" (added "2" and deleted comma).
- 9659/3: Hodgkin lymphoma, nodular lymphocyte predominant, Primary Site Change from C440-C449 to C770-C779.
- 9811/3: Precursor B-cell lymphoblastic leukemia. Abstractor Notes: Deleted, "Pre 2010 ICDO3 code are now a category of neoplasm which includes B Lymphoblastic Leukemia/Lymphoma, NO" and inserted, "For cases diagnosed 2010 and later, code pre-BALL to B-lymphoblastic leukemia/lymphoma, NOS 9811/3."
Multiple Primary Calculator
- 9671 LPL and 9761 WM changed from "New Primary" to "Same Primary".
- 9671 LPL and 9762 Heavy chain disease changed from "New Primary" to "Same Primary."
- 9650/3 and 9684/3 are now "New Primary" and 9684/3 and 9650/3 are now "New Primary."
- 9591/3 and 9650/3 are now "New Primary" and 9650/3 and 9591/3 are now "New Primary."
- 9650/3 and 9591/3 are now "New Primary" and 951/3 and 9650/3 are now "New Primary."
- When 9591 9724 Non-Hodgkin, NOS and EBV positive lymphoproliferative disease of childhood is entered in reverse order, 9724 9591 EBV positive lymphoproliferative disease of childhood and non-Hodgkin, NOS, it now displays Same Primary, rather than New Primary.
- When 9591 9726 non-Hodgkin, NOS and primary cutaneous Gamma Delta T-cell lymphoma is entered in reverse order, 9726 9591 primary cutaneous Gamma Delta T-cell lymphoma and non-Hodgkin, NOS, it now displays Same Primary, rather than New Primary.
- When 9650 9655 classical Hodgkin lymphoma and Hodgkin lymphoma, lymphocyte depletion, reticular is entered in reverse order, 9655 9650 Hodgkin lymphoma, lymphocyte depletion, reticular and classical Hodgkin lymphoma, it now displays Same Primary, rather than New Primary.
- When 9650 9659 classical Hodgkin lymphoma and nodular lymphocyte predominant Hodgkin lymphoma is entered in reverse order, 9659 9650 nodular lymphocyte predominant Hodgkin lymphoma and classical Hodgkin lymphoma, it displays Same Primary, rather than New Primary.
- When 9650/3 and 9680/3 are entered IN ANY order, the answer is now New Primary, rather than Same Primary.
Manual Revisions
Introduction
- Pg 2: Removed from Hematopoietic Working Group list, "Wendy Scharber, RHIT, CTR, Registry Widgets, INC."
- Pg 8: 3rd paragraph now reads as: "When the ICD-O-3 editors assign new codes for a neoplasm..."
- Pg 11: Under "What is new and/or Different in 2010", added a comma in the first sentence after "the histology groupings." The 2nd sentence, now reads: "International Classification of Disease for Oncology."
Multiple Primary Rules
- M3 - All Formats: Inserted new Note 1: "Do NOT use this rule for cutaneous lymphomas." Renumbered existing notes.
- M10 - All Formats: Changed "21 days or more" to "more than 21 days."
- M12 - All Formats: Changed Note 1 to "...when the patient has been treated for the acute disease."
PH Rules
- PH11 - Matrix Only: Changed "Precursor B-ell lymphoblastic leukemia/lymphoma" to "Precursor cell lymphoblastic leukemia, NOS."
- PH12 - All Formats: Bullet 2 "Blastic plasmacytoid dendritic cell neoplasm 9727/3" changed to "Blastic plasmacytoid dendritic cell neoplasm (previously called precursor cell lymphoblastic lymphoma, NOS) 9727/3."
