Question:
Surgery of Primary Site--Skin: For skin primaries diagnosed 1998-2002, what is the difference between code 40 [Wide excision or re-excision of lesion or minor (local) amputation, NOS] and 50 [Radical excision of a lesion, NOS]?
Discussion:
Answer:
Codes 40 and 50 are not in the scheme for 2003 forward. See history for coding cases diagnosed 1998-2002.
History:
For cases diagnosed 1998-2002: There must be at least a 1 cm margin of resection for surgical procedures described in codes 40 and 50.
Code 40 is used when there is a wide local excision or a "local" amputation of a digit, an ear, a lip, etc.
Code 50 is used when an extensive excision is done that removes regional tissue/organs such as an amputation of a leg, the removal of the skin (primary), deep muscle, bone, etc. A radical excision is not a common surgical procedure for skin cancers.
