Discussion:
A patient was not diagnosed prior to autopsy. The autopsy diagnosis states that this is adenocarcinoma of the prostate without capsular invasion.
Should clinical extension be coded to clinically inapparent, NOS (10) and pathologic extension be coded to no prostatectomy done within first course of treatment (97)?
Answer:
History:
EOD-Clinical Extension/EOD-Pathologic Extension: Code the EOD-Clinical Extension field to 90 [unknown if extension or metastasis]. If you code this case to 10, clinically inapparent, it would give the impression that the case had been clinically staged and then it would not be an autopsy-only case.
If it is an autopsy-only case, the information from the autopsy should be coded into the EOD-Pathologic Extension field, because the patient underwent far more than a prostatectomy.
In this specific case, code the EOD-Pathologic Extension field somewhere in the range of 20-34.
