Per PH Rule22, code the primary site to C779 [lymph nodes, NOS].
Rule PH22 is a default rule for lymphomas that is used when there is no other information regarding the primary site and the Heme DB does not indicate a primary site under its Primary Site(s) section. Rule PH27, code the primary site to unknown [C809], does not apply. Only use C809 [unknown] as the primary site when there is no evidence of lymphoma in lymph nodes AND the physician documents that the lymphoma originates in an organ(s).

For cases diagnosed 2012 and later, access the Hematopoietic Database at http://seer.cancer.gov/tools/heme/. Click on Hematopoietic Project. Click on Hematopoietic and Lymphoid Database. The 2012 Hematopoietic Coding Manual (PDF) will appear which indicates the correct version of the program is available now for query.
Code the primary site to lymph nodes, NOS [C779]. The steps used to arrive at this decision are as follows:
Enter in the Heme DB to find the histology. Click on the SEARCH button. Ensure that the term "Mantle cell lymphoma" [9673/3] is highlighted on the screen.
Review the PRIMARY SITE(S) section. No typical primary site is indicated but it refers one to the ABSTRACTOR NOTES and MODULE 7. Scroll down to the ABSTRACTOR NOTE information. While no specific primary site is listed, there is an indication that the bone marrow, liver and GI tract involvement occurs in a very high percentage.
Click on the 2012 HEMATOPOIETIC CODING MANUAL (PDF). Once in the manual, select the Primary Site and Histology Coding Rules. The rules are intended to be reviewed in consecutive order within the applicable Module.
Go to MODULE 7: Primary Site Rules for Lymphomas Only 9590/3-9729/3 PH25-PH37. Stop at the first rule that applies to the case you are processing. Code the primary site to lymph nodes, NOS [C779].
